I have been looking for the above board for a friend of mine, any suggestions where it might be found? Have tried all my normal channels to no avail. I believe the part number is 5100004750.

Replacing whole circuit boards is a last ditch method, and no manufacturer makes spare circuit boards anymore. If its just a broken jack you need then why not replace just that broken jack. If there is an unused jack on the board you can swap the damaged one with the unused one. If that;'s not possible contact Roland for the broken part. You can also look up Digikey, they carry replacements for most jack types.
